I recognize that any material concerning all and any clients of Avondale House or persons making contact with Avondale House is to remain in the strictest confidence. No material concerning a particular client may be released to or discussed with anyone outside of Avondale House staff members without prior knowledge and written consent of the client, parent, or legal guardian.

I acknowledge that I have downloaded or received a copy of the Volunteer Handbook and have read and understand the contents of the handbook, media release, and release of liability. I will act in accord with these policies and procedures and grant the permissions and releases described therein. I understand that if I have questions or concerns at any time about the handbook or standards of conduct, I will consult the volunteer coordinator.
